Short-Term Independent Study Program

For students that have voluntarily enrolled in the Short-Term Independent Study Program you will find the rubric for the attendance form here. Please fill out the attendance form each day you are absent and with the work you have done. If you do not fill it out completely, you will not be awarded the attendance for short-term and your absences will remain unexcused or excused depending on notes provided.
