If you are at least 16 years old and weigh at least 135 lbs, or you are 17-18 years old and weigh at least 110 lbs, and would like to donate blood on September 18th, please complete this form.
Please note, if you are 16, you will need to turn in a signed parent consent form in order to donate blood. You can pick up a parent consent form in room 133 or at the front desk.
If you plan on donating, please make sure that you eat and drink plenty of fluids on September 18th before and after donating.
ASB will send you a summons on September 18th for the time you are to donate. When you come to donate, please bring your POLAHS ID, and if needed your signed consent form.
