9th Grade Lottery Application Form 2019-2020

Please complete the requested information as it applies to your student. Lottery receipts will be emailed when a completed form is submitted. Forms will be accepted from December 3, 2018 at 7:30am - February 15, 2019 at 4:00pm.

The incoming freshmen lottery will be held at POLAHS on February 22, 2019 at 4:00pm.

Lottery Application forms may also be printed and submitted in person (by a parent or legal guardian) or by mail. For questions, admissions office hours are Monday through Friday from 8:00 am to 4:00pm. 

Note: Applications will not be accepted during regularly scheduled school break times 10:00-10:30am and 12:25-1:00pm.
