If you are at least 16 years old and weigh at least 135 lbs, or you are 17-18 years old and weigh at least 110 lbs, and would like to donate blood, please complete the attached form.
Please note, if you are 16, you will need to turn in a signed parent consent form in order to donate blood. You can pick up a parent consent form in room 133 or at the front desk. You must bring this signed form to the blacktop on 9/15 when you donate blood; if you do not have this signed form, you will not be able to donate blood. You can not turn this form in early to Ms. McG in 133; you must bring this form with you to the blacktop on 9/15 when you are called to donate and turn it into the representatives from Cedars at check in.
If you plan on donating, please make sure that you eat and drink plenty of fluids before and after donating.
ASB will deliver a summons on the day of the blood drive for the time you are to donate. When you come to donate, please bring your POLAHS ID or CA ID, and if needed, your signed consent form.
